Where is the entire world rendered from space to surface?

That's all good and dandy... until you want to do large detailed areas, an entire to-scale city or a vast forest for example. It is impressive how realistic things are getting though, in my opinion we still have a fair few barriers to cross until we are finally there though namely soft body self collisions (so characters arms etc don't clip through the bodies), real time reflections on every reflective surface, reactive smoke and fire, mass and realistic liquid physics, dynamic human-sounding voice (characters can speak your name or any other procedurally generated dialog) and of course believable AI. I'm pretty sure I've missed a whole bunch of things but those seem to be the most prevalent
